Payment is due at the time of service. We accept cash, debit card, credit card, health savings account (HSA), and flexible spending account (FSA).

 

The services that we provide at Lakeview Wellness Clinic are generally not covered by insurance, therefore, we are not set up to bill insurance at this time. The use of ketamine for mood disorders is considered off-label, and most insurance companies will not provide coverage. We can provide you with a superbill that you can submit to your insurance company. Please check with your insurance company to see if they will provide some form of coverage prior to starting treatment.
